Here, busy ED clinicians can refer patients to VITAS securely via face sheet or mobile form and receive an in-app referral confirmation. *

The app also features a “Call-Me” button which enables 1-click access to a VITAS admissions coordinator; the most up-to-date hospice eligibility guidelines for major disease types; and useful assessment tools such as a Palliative Performance Scale, BMI calculator, and an opioid converter tool.
